No to the team informing another team. Privacy should prevail.
Yes, to the player should inform any team's doctor, and a medical review should be part of the offer made by a team for joining a team. Part of that offer may be a requirement that the prospective member must allow other teams' doctors' medical records be made available before the prospective member may be accepted.
Consider if an athlete played in high school and had a clean medical record. But, out of season an athlete had multiple concussions possibly totally unrelated to their sport. Should a college program invest 4 years of precious-few scholarships in that athlete, if the athlete doesn't divulge information pertinent to their ability to participate?
