I think it is not 100% simply because some people chose other paths, like transferring. It used to be that a significant (but still not large) number of students actually did "flunk out". The system has been changed such that that no longer applies, but if you are not doing well it will cost you lots of money (and time) to finally get a degree.
Where I went to college decades ago, the first thing they told us was that many would not graduate. It was an intended shock.
