Simply tragic. Very painful for all involved.
I recall a friend of my father's driving off a small bridge on his golf cart as he traveled from one home to another. The bridge was less than 3 feet above the tiny creek it crossed, and my father's friend was uninjured, but very embarrassed, for the mishap.
