How consistent has this been? Shaw wasn't listed on the form that covered his first season (2011), but is listed on the form covering 2012 (he made ~$2M, it looks like, Harbaugh made ~$1M his last year). It's possible he wasn't making enough, but then he would've been making <$1M, which I guess is possible, but then that was QUITE a pay bump for the 2012 year, which I guess could make sense since he won the conference that year, but hadn't yet won the Rose Bowl.
When do these come out? I'd be interested to see the one covering 2013.
Oh, also, my dates may be screwed up, the forms are dated with the years I've said but called "Fiscal Year N+1", so I'm not sure what dates they cover. It's possible the 2012 one does cover the Rose Bowl and contract extension if it's something like Oct 2012-Sep 2013.
