Rules of golf say if you don't find it in five minutes it's lost even if it's found later. You have to go back to where you hit your last shot from and take a one-stroke penalty (in addition to the stroke you took when you hit it the first time). This is often referred to as "losing stroke AND distance" and is different from when a ball is hit into a water hazard marked by yellow or red stakes where you are penalized a stroke but don't lose the distance.
So if you hit a tee shot and lose the ball or hit it OB, your second tee shot will be your third stroke. This is how you end up with 7s and 8s on your scorecard - speaking from VAST experience here :)
To improve the speed of play, if you think your ball may not be found or may be out of bounds (same penalty as lost ball), you hit a provisional ball. If you find your first ball, you pick up your provisional. If you don't, then you play your provisional with the appropriate penalties and don't have to go all the way back to where you hit your last shot.
