A four-yard pass where five is needed is almost always the  fault of the passer (and/or perhaps the receiver) and not the play call.  Costello too often checks down to receivers well short of the marker.

Yeah, I sure hope the play isn't designed to get four yards when we need five. I always assumed it was the receiver cutting his route short for one reason or another. And of course, a four yard gain is better than a sack or just throwing the ball into the stands.
