But maybe consider the coaching is hard, and that if you were in the spotlight you'd occasionally make "inexcusable" decisions.

It's not that hard to make the right decision in that situation (snark or no snark). Really.
Well, of course, all decisions that turn out badly are INEXCUSABLE. Say, for example, you need 3 points to win, oh, I don’t know, how about the Fiesta Bowl. You have an eminently makeable field goal, but you would like to be a little closer, so...
... you throw a short pass, and it gets picked off. INEXCUSABLE
... you hand the ball off, and there’s a fumble. INEXCUSABLE
... the QB gets sacked for an 8 yard loss, taking you out of field goal range. INEXCUSABLE
... there’s a holding penalty on a play action. INEXCUSABLE
... you decide not to risk those things, but your kicker misses the field goal. INEXCUSABLE
Also, not sure about the lumping of “you-know-who” with Shanahan on this one. Scarlett up the middle 3 times is almost a guarantee... as would be the cry of INEXCUSABLE that he turtled rather than try to win...
Instead of going through each of your "sky falls" scenarios, I'll just ask you two questions:
1. Assuming you watched the 'Niners game last night, when they got the ball on their own 20 yard line with 1:45 left and Seattle having no timeouts, did you have an opinion as to what SF should do, or do you just watch and say "Hey, this is an interesting game, I wonder what will happen next?" If the former, did you feel strongly about it? Was your feeling, if any, informed by the overall standings and/or the flow of the game up till that point?
2. Do you really think you-know-who would have been excoriated for letting Andrew Luck try to score a TD or move into real "gimme" FG range at end of Fiesta and then there was a turnover? Really? Dude had been going through Okie State D like $hit through a goose all day; your returning-from-injury kicker had already missed on one FGA; and 37 yards is no gimme. I don't think anyone would have called letting Luck attempt to score/get closer in final 1+ minute "inexcusable." C'mon now.
And for BC, there's a difference between calls where reasonably (or even marginally) informed minds can differ - going for it on fourth down and inches vs. trying the GW FG in OT last night - is a classic example. Trying the FG and missing didn't work out, but it wasn't a bad decision, let alone inexcusable. Yes, fans complain all the time about bad "play calls." But the number of truly indefensible calls is very small (and should be zero). And if Shanahan has a lot going on real time, one of those things should be having someone on the headsets who isn't calling plays who can chime in with a reality check. I have no doubt Shanahan has a big ego and great confidence in his play calling and wants to win every game, especially with his O, to confirm his brilliance. But you know what, sometimes trying to show that off isn't the best thing for the team.
Last night at the end of OT, Shanahan did not do what was best for the team.
