That was the late hit on the lineman? As a neutral fan I thought it sounded dubious until I saw the replay, the defensive player had clearly stopped playing when the lineman took him out, so I don't think the line that he couldn't hear the whistle is accurate. As Collinsworth was going on about it I was thinking "eh, I dunno, it's marginal but it's not wrong."
Re: penalties, I think any Niners fan will gladly trade that penalty for the non-call holding/DPI on Seattle's last series, that one pretty easily could've justified a flag.
At any rate, it was an entertaining game. My wife came downstairs to read her book near me and ended up being as engrossed in the ending as I was, she typically doesn't watch football at all (outside of attending Stanford games).
I think it was a late hit on a defensive back. Collinsworth was saying that you couldn't hear the whistles on the field. Gardner agreed (two separate articles in the link below):

The Niners RBs are so good, that our center (and other linemen) are downfield blocking their DBs. Garland didn't hear the whistle, assumed the play was ongoing. And BTW...the non-holding DPIs were legion, throughout the game.
