Might be the difference between "sign this so that we can disclose your injuries" and "decide whether it would be ok with you for us to disclose your injuries. If you don't want us too, we will simply say you are unable to play due to injury. If you don't mind and sign the consent, we will let reporters know as much you are comfortable with us saying."
