Indeed, one of the reasons I will question a call is when the ref is blocked from seeing the contact but still makes the call.

It is undoubtedly true that many calls are made based not on what the ref actually saw but rather on the basis of what the ref believes was the "most probable" interpretation of the little he did see. Often fouls are called because the ref expects that in a given situation player A is pretty much for sure will foul player B, so he calls what he expects, not what actually happened.
