Who fumbled for the 49ers and who threw the pass that the receiver caught and fumbled?

A vivid memory from my youth - circa 1963??  Though I grew up in Denver, so not a 49er fan at that time.  I think the QB was George Mira.  I feel like I should remember who fumbled, but I can't shake that name out of the cobwebs.
