I have no idea which teammate or what incident, but the suggestion that the death of a teammate could have been triggered by an incident involving another teammate might make it even more difficult to process one's grief by jumping right back into the team environment. I can only imagine how difficult it must be for other teammates who knew and loved Katie to continue as "normal" as well. I hope they are all getting the support they need. Stanford has a lot more mental health resources than they used to (and every student ID now has a number on the back that students can call). But, as Andrea suggests in her post, there needs to be a strong culture among coaches, professors, deans/administrators, etc. that accommodates treatment for mental health issues the same as more overt injuries/illnesses.
