I don't think the scouting of high school players is any looser than in football or basketball, but I think the business of putting rankings out on the web is much less developed. That leads to a lot of misses in what is available.
One thing to note with Perfect Game class ranking is that Stanford's classes always suffer in the ranking system by being small. This is a general feature of Stanford recruiting across sports. As noted in football recruiting ranking discussions, size itself can be a plus in the value of a class, but it is much less in baseball than football given the number of people who play.
One note of clarification. Graham wasn't top 500, he was 500+. What Perfect Game does is rank 499 guys, then dumps everyone else they list as "500". As far as I can tell, everybody with the 500 designation is listed in random order.
