However, there already have been two occasions on which a team won three straight NFL championships. Which teams were they, which years did they three-peat, who were their coaches, and what were their home stadiums?

Green Bay Packers is one for sure - 1966, 1967 (first Super Bowl, although not certain it was known as "Super Bowl" at the time - maybe just NFL-AFL championship game), & 1967.  Vince Lombardi was the coach, Lambeau Field the home stadium (and I think they had occasional home games at County Stadium in Milwaukee in those days).
