The problem has little to do with passive voice and lots to do with the UNHCR. It’s members include many of the worst violators of human rights, such as Syria, Sudan, and Afghanistan. The UNHRC has been a joke at least for my entire adult life, which is a long time.

There are plenty of examples outside UNHCR.  For example, here’s the chairman of the FAA saying “mistakes were made” in the context of the 737 Max.
