Who are the only two people ever awarded both a World Series ring and a Super Bowl ring?

There's no vote, the owner decides.
In the Eddie D era secretaries, P.R. staff, equipment guys, basically everyone got a ring I believe. Great for morale.

Don't know, but seems like it probably is announcers like you said. Jack Buck definitely got a ring when the Cardinals won the WS, Joe Buck has talked about it, although I don't know where he would have won a Super Bowl ring. The great Bill King perhaps, Athletics and Raiders were both champions during his tenures.
